Substance misuse outreach worker

Experience carrying a caseload of socially excluded clients is essential. Experience engaging clients through outreach is essential. Experience with substance misuse clients is a benefit.

A local service is looking for an outreach practitioner to engage with clients who have finished drug treatment programs or who are looking to re-engage with society. The role will involve assessing clients through out reach work, assessing their levels of treatment and needs and designing care plans to refer them to local treatment services or help them engage in leisure activities, college or other educational courses or helping them return to work.
