Social Worker

The candidate must be a qualified social worker and have experience working with substance misuse clients. The applicant will provide a social work and care management service as part of a multi-disciplinary team delivering assessments, prescribing and intervention services to adults with substance misuse problems. In addition to this the candidate will be conducting assessments, designing and developing care plans as well as carrying a case load of clients. The post holder will also be required to produce written reports on behalf of service users to external agencies and provide reports on the overall development of the project.
