Care Coordinator Post

A new opportunity has arisen in the West London Area for Care Coordinator. A suitable candidate would have experience of working to U.K legislation and have practice codes. Experience of working with in physical disabilities or hospital team is essential and student placement experience will be accepted. A social work qualification would be beneficial but is not essential. Candidates will be expected to conduct care management duties including reviewing care packages.
