Hospital Social Worker

A new opportunity has arisen in the East London area for a Hospital Social Worker post. A suitable candidate will have experience of working with in a hospital setting and experience of working with older people is essential. Knowledge of the delayed discharge act is essential and mental health experience beneficial but not essential. The candidate would be based in two hospitals in the area conducting care plans, writing and review care packages and working closely with clients who are to be discharged in to the community or moved to a care home. A Social work degree, DipSW, CQSW, CCETSW or any other GSCC recognised qualification is essential.
