Older Person Hospital Social Worker

An ongoing opportunity for a hospital social worker post has arisen in the North London area, to work with older people due to leave hospital. Duties will include carrying out assessments for service user’s needs for residential or community based care plans and care packages. A Social work degree, DipSW, CQSW, CCETSW or any other GSCC recognised qualification is essential. Experience in working in a hospital or older people social work team is essential. Knowledge of a delayed discharged act will be a befit.
