GP shared care practitioner

Experience performing assessments and designing care plans for substance misuse clients is essential. Knowledge of opiates and harm minimisation is also essential.

A local organisation is looking for a drugs worker to take on a caseload of primarily opiate clients in a GP shared care service. The role will involve assessing clients from self referrals and a drop in needle exchange with a view to designing care plans to include signposting to GPs for substitute prescribing of methadone and subutex. After the initial contract this role has the opportunity to go permanent.
