Care Co-ordinator, Housing Liaison
The candidate must have worked with and be familiar with the needs of the elderly or older adults with disabilities. The candidate must have worked within sheltered housing or managed a sheltered housing scheme. Further to this the candidate must be familiar with benefit checks and loan or grant applications. Additionally the candidate must have an awareness of the needs and requirements of informal carers and an understanding of public sector housing.
This role involves liaising between housing and community services, conducting assessments on older adults leaving hospital with a physical disability. This will include home visits to ascertain the suitability of social housing and may require the candidate to move clients into sheltered accommodation.
