Shared Care Practitioner

Experience case managing class a drug users is essential. Experience advocating prescriptions or titrating clients on prescribed substitutes would be a benefit. A car driver would be a benefit but is not essential.



A local service is looking for a shared care worker to work across a number of local GP surgeries case managing substance misuse clients. The role will involve taking on an existing case load of opiate users on methadone or subutex, adjusting prescriptions and reviewing careplans for clients across a number of surgeries. Although this role is temporary there is the possibility of going permanent after the end of the contract. This role also offers the opportunity for flexible hours across 3 to 5 days a week
