Leaving Care Personal Advisor

Experience case managing pathway plans for young people in a leaving care setting is essential. A qualification in youth work to a minimum of NVQ level 3, degree or diploma in social work is essential. Experience with the CAREFIRST I.T. system would be a benefit.


A local service is looking for a personal advisor to assess the needs of young people in the leaving care system. The role will involve the implementation of pathway plans, working with adolescents (aged 16 – 21) entering adulthood on a one to one basis and in groups to help them integrate into independent living. Experience case managing pathway plans for young people in a leaving care setting is essential. A qualification in youth work to a minimum of NVQ level 3, degree or diploma in social work is essential. This role will involve working with people in a leaving care setting.
