QSW physical disabilities/substance misuse

Qualified social worker (GSCC registered) with experience with physical disabilities, substance misuse and homelessness essential.

A local counsel is looking for a qualified social worker to carry out needs led assessments on service users with physical disabilities at risk of homelessness and with possible substance misuse issues. The role will involve liaising closely with local authorities, NHS as well as private and voluntary organisations in the implementation of care management.
